Requesting Mental Health Records
You have the right to request a copy of your mental health records. Records will be provided according to Texas law and applicable privacy regulations. You may request records via email, in writing, or through the messaging feature of your secure client portal.

PLEASE NOTE: All requests for records must be unambiguously stated; in other words, the request must clearly state that the purpose of the communication is to request a delivery of your mental health records and must not contain other questions pertaining to your records or information unrelated to your records. You must sign a release of information (ROI) prior to receiving the requested records. A fee of $2.00/page of client records will be assessed to cover copy/printing charges. Please make your records request with plenty of advance notice of requiring the records and allow 2 weeks for delivery.
